After years of effort in your home country, the process of validation (Approbation or Berufserlaubnis) and long shifts in hospitals in Berlin, Munich, Frankfurt, or Cologne, it's easy to postpone financial planning. However, understanding how retirement for doctors in Germany works from your first contract is essential to avoid losing thousands of euros or mistakenly paying duplicate contributions.

What is Ärzteversorgung and how does it work?

In Germany, doctors are part of the regulated liberal professions (kammerfähige freie Berufe). By law, they are not required to remain in the general public pension system (Deutsche Rentenversicherung - DRV), but have their own professional pension fund: the Ärzteversorgung, managed by the respective state medical chambers (Landesärztekammern).

Each federal state has its own organization (for example, the Bayerische Ärzteversorgung in Bavaria or the Nordrheinische Ärzteversorgung in North Rhine-Westphalia). Although each fund has its own statutes (Satzung), they all share one principle: the contributions from doctors are managed through partially funded plans (offenes Deckungsplanverfahren), which has historically resulted in higher pensions compared to the standard state system.

Ärzteversorgung vs. Rentenversicherung: The Exemption Process

When you start working as an employed doctor (angestellter Arzt) in a clinic or private practice, your employer will automatically register you with the Deutsche Rentenversicherung. To allocate your contributions to the Ärzteversorgung, you must formally request an exemption.

The Exemption Application (Befreiungsantrag)

To avoid contributing in two places at the same time, you must submit the exemption application (Antrag auf Befreiung von der Rentenversicherungspflicht) according to § 6 paragraph 1 number 1 of the SGB VI. This process is done digitally through your medical chamber's portal.

  • Strict deadline: You have exactly 3 months from the first day of your medical work activity to submit the application. If you do it on time, the exemption will take effect retroactively.

  • Risk of delay: If you submit the application late, your previous contributions will be trapped in the DRV and the exemption will only be valid from the date of receipt.

  • Change of employer: Each time you change hospitals or positions (for example, if you move from Stuttgart to Düsseldorf or change departments), you must submit a new exemption application for your new contract.

The contribution rate is around 18.6% of your gross salary (up to the contribution assessment ceiling or Beitragsbemessungsgrenze), of which your employer pays 50% and the remaining 50% is deducted from your payroll.

Can I withdraw my Ärzteversorgung if I return to my home country?

One of the most frequently asked questions among Spanish-speaking doctors is what happens to the accumulated money if they decide to return to Spain or Latin America after a few years of practicing in Germany.

The answer is clear: you do not lose your acquired rights (Anwartschaften). Upon reaching the statutory retirement age (Regelaltersgrenze, generally at 67 years), you can request the monthly payment of your medical pension from anywhere in the world. The amount will be transferred to your international bank account or a European account.

Depending on the statutes of each medical fund and your nationality, in certain cases of short stays (less than 60 months of contributions) and definitive exit from the European Union, there is a technical possibility to request a refund of your own contributions (Beitragserstattung), although in fiscal and profitability terms it is usually more convenient to retain the right to future pension.

Blind Spots of the Ärzteversorgung: Disability Protection

Although the Ärzteversorgung offers a solid foundation, it has significant limitations that you should be aware of, especially regarding occupational disability (Berufsunfähigkeit).

The disability coverage of the public medical fund usually requires a total medical incapacity of 100% to practice medicine and the surrender of your medical license (Approbation). If you suffer from a musculoskeletal, neurological illness, or chronic stress that prevents you from operating or doing shifts, but you can still perform administrative tasks or medical assessments, the Ärzteversorgung may deny the pension for professional disability.

For this reason, the vast majority of doctors in Germany take out a private supplementary occupational disability insurance (Berufsunfähigkeitsversicherung) tailored to their specialty, as well as private retirement plans (such as the Basisrente or Rürup pension) to optimize the tax burden (Steuerersparnis) during their higher income years.
