Working as a fee-based doctor can offer flexibility and attractive billing, but the amount on an invoice does not equate to a net salary. Before accepting an on-call duty or collaboration, you need to understand what portion you must set aside for taxes, social security, and insurance.
This guide on fee-based doctor taxation for Hispanics summarizes the essential points for doctors with a professional license (Approbation) or a temporary practice permit (Berufserlaubnis) in Germany.
What Does Working as a Fee-Based Doctor Mean?
A fee-based doctor (Honorararzt) provides medical services and receives an agreed remuneration per hour, shift, or project. However, the name of the contract does not determine whether a self-employed activity (Selbstständigkeit) truly exists. Authorities analyze how the work is performed in practice.
Since several rulings by the Federal Social Court (Bundessozialgericht) on June 4, 2019, fee-based doctors integrated into the usual organization of a hospital are often considered employees subject to social security contributions (sozialversicherungspflichtige Beschäftigung). This can occur when the hospital sets schedules, assigns patients, provides all materials, and subjects the doctor toDealing with tax procedures in another language can be overwhelming, especially during your first year in Germany. However, filing your tax declaration in Germany for Spanish speakers can help you recover overpaid money and better organize your finances.
This guide explains how to use ELSTER, what documents you need, and what expenses you might be able to declare, whether you live in Berlin, Munich, Frankfurt, Cologne, Dusseldorf, Stuttgart, or any other German city.
What is ELSTER and who must file a declaration?
ELSTER, an abbreviation for electronic tax declaration (Elektronische Steuererklärung), is the official portal of the German tax administration. Through Mein ELSTER, you can submit your income tax declaration (Einkommensteuererklärung) to the relevant tax office (Finanzamt) free of charge.
For many employees with a single payroll, filing the declaration is voluntary. This is called assessment upon request (Antragsveranlagung). In other cases, there is an obligation to declare (Pflichtveranlagung), for example:
If you simultaneously had several employers or tax class VI (Steuerklasse VI).
If a couple with joint assessment used certain combinations of tax classeSpecializing can open the door to more advanced clinical roles, better shifts, and potential salary improvement. But before enrolling, it's wise to calculate the real cost, understand the available support, and prevent training from destabilizing your finances.
How does nursing specialization work in Germany?
Nursing specialization in Germany is usually achieved through advanced professional training (Fachweiterbildung). It is aimed at nursing professionals (Pflegekräfte) who already have a recognized qualification and wish to work in a specific area.
Common options include intensive care and anesthesia (Intensivpflege und Anästhesie), oncology (Onkologische Pflege), psychiatry (Psychiatrische Pflege), nephrology (Nephrologische Pflege), hospital hygiene (Hygienefachkraft), and nursing unit management (Stationsleitung).
Admission requirements and duration
Requirements vary depending on the federal state, hospital, and training center. Typically, professional recognition (Anerkennung) as a nurse, an employment contract, and prior experience in the chosen area are required.
Specialized training in intensive care (Fachweiterbildung Intensiv- und Anästhesiepflege) usually lasts around two years part-time. According to the model followed by numerous centers, it may include approximately 720 theoretical hours and 1,800 practical hours. You must confirm the exact program with the provider, as regional regulations and educational plans are not identical throughout Germany.
Before enrolling, ask if the qualification has state recognition (staatliche Anerkennung) or is based on the recommendations of the German Hospital Society (DKG-Empfehlungen). This can influence its acceptance by other employers.
How much does it cost to specialize as a nurse?
There is no national fee. Some hospitals fully cover the training of their staff, while private centers may charge approximately between 3,000 and 9,000 EUR for the complete program. This figure is only a reference: always request a written offer.
In addition to tuition, include in your budget:
Registration and examination fees.
Books, digital materials, and uniform.
Transport, accommodation, or meals during in-person modules.
Unpaid study hours.
Loss of allowances for night shifts, Sundays, or public holidays.
Childcare, when necessary.
Example of a realistic budget
Imagine a nurse in Cologne with a tuition fee of 6,000 EUR, transport and material costs of 1,200 EUR, and an estimated loss of 2,400 EUR in shift allowances. The total economic cost would be 9,600 EUR.
If the hospital contributes 4,000 EUR, 5,600 EUR would remain to be financed. Spread over 24 months, this would be about 233 EUR per month, before considering possible support or tax advantages. This calculation shows why you should not focus solely on the price advertised by the academy.
Specialization also does not automatically guarantee a specific salary. In institutions subject to the collective agreement for public service (TVöD-P), classification may improve when the position requires the qualification and corresponding duties are assigned. The qualification alone does not oblige the employer to change your salary group.
Support for nursing specialization
Employer, public aid, and tax deduction
The first conversation should be with your employer. Many hospitals and care homes offer full or partial funding, study hours, time off (Freistellung), or maintenance of certain allowances. Ask for everything to be reflected in a training agreement (Fortbildungsvereinbarung).
Read any repayment clause (Rückzahlungsklausel) carefully. Some employers require reimbursement of a portion of the cost if you voluntarily resign shortly after completion. The duration of the commitment and the progressive reduction of the debt must be reasonable for the amount financed.
You can also review these aids for nursing specialization:
Advanced Training BAföG (Aufstiegs-BAföG): Certain recognized programs may receive a combination of grants and loans. For eligible measures, a portion of the costs may be granted as non-repayable aid and another through a loan from the public bank (KfW). The competent office must first check if your course meets the requirements.
Training voucher (Bildungsgutschein): The Federal Employment Agency (Bundesagentur für Arbeit) or the job center (Jobcenter) may finance approved courses when the training improves employment prospects or prevents unemployment. It is not automatically granted to all employed individuals.
Educational leave (Bildungsurlaub): Some federal states allow several working days to be used for recognized training. This leave typically provides time, not necessarily tuition payment, and rules vary by region.
Tax deduction: Costs paid by you can be deductible as professional expenses (Werbungskosten) in your tax declaration (Steuererklärung). Keep invoices, tickets, receipts, and the course schedule. A deduction reduces taxable income; it does not mean the tax office refunds 100% of the expense.
Do not combine aids without informing the corresponding institutions. If the hospital pays an invoice, that same amount cannot be presented again as your own expense.
Financial plan before starting
Prepare three scenarios: full employer funding, partial funding, and self-payment. Calculate how much money you will have left each month if your additional shifts decrease and create a reserve for unforeseen events. It is advisable not to rely on future shifts to pay fixed commitments.
It is also advisable to review your income protection. A prolonged incapacity can make both clinical work and training payments difficult. Occupational disability insurance (Berufsunfähigkeitsversicherung) can offer an agreed income if, for health reasons and under the contract conditions, you can no longer practice your profession to the defined degree. In nursing, it is important to analyze this in advance, because age, health status, tasks, and the insured amount influence acceptance and the premium.
Also review your private liability insurance (Privathaftpflichtversicherung). This protects against certain damages caused in private life, but does not automatically replace the employer's professional liability coverage (Betriebshaftpflichtversicherung). The specific conditions should always be checked.
A good specialization can improve your prospects in Berlin, Frankfurt, Munich, or any other region, but it must fit both your career and your budget.

For the 2024 declaration, the general mandatory deadline without a tax advisor was July 31, 2025. When an authorized tax advisor (Steuerberater) or a wage tax assistance association (Lohnsteuerhilfeverein) is involved, the general deadline for 2024 is extended until April 30, 2026. If the declaration is voluntary, you usually have four years: the one for 2021 can be submitted until December 31, 2025.
How to file your Steuererklärung with ELSTER
1. Create and activate your account
Go to www.elster.de, select the option to create a user account (Benutzerkonto erstellen), and register with your tax identification number (Steueridentifikationsnummer). Activation may include an email and a letter sent to your home address, so it's advisable to register before the deadline.
2. Gather your documents
Before you start, prepare your annual wage tax certificate (Lohnsteuerbescheinigung), your tax number, IBAN, insurance certificates, and proofs of work-related expenses. If you activate the electronic receipt retrieval (Belegabruf), you can import data already communicated by employers, insurers, and other institutions.
On the platform, you will find the main form (Hauptvordruck), the appendix for employment income (Anlage N), and additional forms such as the appendix for provisions and insurance (Anlage Vorsorgeaufwand). Always review the imported data: just because it appears automatically does not mean it is complete.
3. Enter expenses and check the result
ELSTER performs a preliminary tax calculation before submission. Invoices are usually not attached initially due to the obligation to retain receipts (Belegvorhaltepflicht), but you must keep them in case the tax office requests them. After processing, you will receive the tax assessment (Steuerbescheid), usually in electronic format and, depending on your settings, also by mail.
Expenses you might be able to deduct in Germany
Employees automatically receive a lump sum for work-related expenses (Arbeitnehmer-Pauschbetrag) of 1,230 EUR annually for 2024 and 2025. Itemizing your expenses becomes particularly relevant when the total exceeds this amount.
Commuting to work: The distance allowance (Entfernungspauschale) is 0.30 EUR per kilometer for the first 20 kilometers and 0.38 EUR from the 21st kilometer onwards. It is calculated based on the single distance between your home and your usual place of work, not the round trip.
Working from home: The daily home office allowance (Homeoffice-Pauschale) allows you to declare 6 EUR per day, up to a maximum of 1,260 EUR annually, equivalent to 210 days.
Professional equipment: Computers, desks, specialized books, or tools can be considered work equipment (Arbeitsmittel) when they have a demonstrable professional connection.
Training: German language courses directly linked to employment, training, and certain studies can be included as work-related expenses (Werbungskosten). A general integration course does not always meet the requirements.
Insurance and provisions: Contributions to health and long-term care insurance (Kranken- und Pflegeversicherungsbeiträge) are usually communicated electronically. Other premiums may be considered provision expenses (Vorsorgeaufwendungen), subject to requirements and tax limits.
Unreimbursed medical expenses could also count as extraordinary burdens (außergewöhnliche Belastungen), but they only have a tax effect when they exceed the personal reasonable burden (zumutbare Belastung), which depends on your income and family situation.
Relocation and refund during the first year
To deduct relocation expenses in Germany, the move must be primarily work-related. This can be relevant if you arrived from another country to start a job, changed cities for a job offer, or significantly reduced your daily commuting time.
Professional relocation expenses (Umzugskosten) can include transportation, trips to find housing, temporary double rent, and other costs not reimbursed by the employer. For moves carried out from March 1, 2024, the lump sum for other relocation expenses (Umzugskostenpauschale) is 964 EUR for the eligible person and 643 EUR for each additional household member. The specific application depends on the circumstances and does not allow declaring costs already reimbursed twice.
How much tax can you get back during your first year? There is no guaranteed amount. As a reference, data from the Federal Statistical Office (Statistisches Bundesamt) for employee declarations corresponding to 2020 showed an average refund of approximately 1,063 EUR among those who received a refund.
Your result may be higher or lower. Starting work mid-year, undertaking a professional relocation, maintaining two households for work (doppelte Haushaltsführung), or having high work-related expenses usually increases the possibility of a refund. A payment due may also arise, especially when there were benefits, secondary income, or insufficient withholdings.

Finanzen Campuzano — Official WebsiteMarrying a German national is an emotional decision, but it also triggers legal, tax, and financial changes that should be understood before signing. Proper preparation can save you delays, unexpected taxes, and problems with your residency or insurance.
This guide explains the essential points of getting married in Germany as a Hispanic person, from documents to changing your tax category.
Documents for marrying a German national
In Germany, legally binding marriages are performed at the Civil Registry Office (Standesamt). A religious or symbolic ceremony can complement the wedding, but it does not replace the civil act.
The first step is to contact the Civil Registry Office responsible for the residence of one of the engaged parties. Exact requirements depend on your nationality, marital status, and country of birth, so there isn't an identical list for all Hispanic individuals.
Documentation usually requested by the Civil Registry Office
Valid passport or identity document.
Full or complete birth certificate.
Extended Certificate of Registration (erweiterte Meldebescheinigung), usually indicating marital status.
Certificate of Marital Capacity (Ehefähigkeitszeugnis), when the country of origin can issue it.
Final divorce decree, previous marriage certificate, or death certificate, if applicable.
When the country of origin does not issue an equivalent certificate, an exemption from the Certificate of Marital Capacity (Befreiung vom Ehefähigkeitszeugnis) can be requested. The file is usually reviewed by the Higher Regional Court (Oberlandesgericht).
Foreign documents may require an apostille (Apostille) or legalization, in addition to a translation performed by a sworn translator (beeidigter Übersetzer). The Civil Registry Office decides which documents it accepts, their maximum age, and whether originals are needed. Therefore, it is advisable not to commission translations before receiving your personalized list.
Administrative fees vary depending on the federal state and international complexity. Registration and review can exceed 100 EUR, not including foreign certificates, apostilles, or translations. If a divorce occurred outside Germany, a procedure for the recognition of a foreign judgment (Anerkennung einer ausländischen Entscheidung) may also be necessary.
Legal Effects of Marriage
If no other agreement is signed, the community of accrued gains (Zugewinngemeinschaft) regime usually applies. This does not mean that all assets automatically become common property. Each spouse retains their assets, but the increase in assets accumulated during the marriage may be compensated upon termination of the regime due to divorce or death.
To regulate properties, family businesses, inheritances, debts, or assets in different countries, a marriage contract (Ehevertrag) may be advisable. This must be formalized before a notary (Notar). For international couples, it is especially important to determine which legislation applies, as it may depend on the couple's choice or their first common habitual residence (gewöhnlicher Aufenthalt), and not solely on the country where the wedding took place.
They can also choose a marital surname (Ehename), although it is not mandatory. From May 1, 2025, German legislation offers more possibilities for forming double surnames. In binational marriages, the rules of the country of origin must also be reviewed.
Residency and Nationality
Marrying a German national does not automatically grant citizenship or a permanent residence permit. The foreign spouse can apply, depending on their situation, for a residence permit (Aufenthaltserlaubnis) for family reunification (Familiennachzug) at the Foreigners' Office (Ausländerbehörde). In certain cases, basic German language skills at A1 level are required, although legal exceptions exist.
Naturalization (Einbürgerung) may become possible for the spouse of a German national after three years of legal residence and two years of marriage, provided that all other current requirements are also met. It is advisable to check the specific case because nationality, residence history, and family situation can change the procedure.
Changing Steuerklasse upon Marriage
After marriage, the tax administration usually registers both spouses in the IV/IV combination of the wage tax category (Steuerklasse). The change is transmitted via electronic wage withholding data (ELStAM), but it is advisable to check the next payslip.
Couples can request another combination from the Tax Office (Finanzamt), usually through the tax portal (Mein ELSTER):
IV/IV: usually suitable when both have similar incomes.
IV/IV with factor: the factor method (Faktorverfahren) distributes withholding more precisely according to estimated income.
III/V: can increase the net monthly income of the spouse with the higher salary, but also lead to insufficient withholding and a possible amount to pay when filing the declaration.
The tax category alone does not determine the final annual tax. It only modifies how much is withheld each month. The final result is calculated through the declaration and, if the conditions are met, joint assessment (Zusammenveranlagung) with the spouse splitting system (Ehegattensplitting).
This system usually yields a greater benefit when there is a significant difference in income. If both earn similar amounts, the savings may be reduced. When the marriage takes place before December 31, joint assessment can usually be requested for the entire tax year. A tax advisor (Steuerberater) can calculate which option is suitable, especially if there is foreign income, self-employment, or properties outside Germany.
Insurance and Financial Planning After the Wedding
Marriage can also change family protection. If one spouse belongs to the public health insurance (gesetzliche Krankenversicherung), the other could access family insurance without additional contributions (Familienversicherung) when their income remains within legal limits. These limits are updated periodically. In private health insurance (private Krankenversicherung), each person usually needs their own contract.
Coverages worth reviewing together
Private liability insurance (Privathaftpflichtversicherung), because a couple or family rate can replace two separate contracts.
Household contents insurance (Hausratversicherung), adjusting the sum insured if you combine households.
Term life insurance (Risikolebensversicherung), especially if there are children, a mortgage, or financial dependence.
Occupational disability insurance (Berufsunfähigkeitsversicherung), to protect family income if one can no longer practice their profession.
It is also a good time to update beneficiaries, bank powers of attorney, wills, and emergency contacts. If there are children from previous relationships or assets in different countries, it is advisable to consult a lawyer or notary specializing in international family and inheritance law.
